A exciting opportunity has arisen for an experienced nurse with experience in A&E, Primary care or Case Management, within a new 24/7 Fast Response Nursing Service in the South Yorkshire area.

The service will run in conjunction with existing services of a GP Practice, Out of Hours Service and Walk In Centre all based in a community health centre. The emphasis for the new service will be on identifying and supporting patients in their own homes providing initial contact, clinical assessment, care delivery and where appropriate avoiding hospital admission.

The role involves:-
-Providing quality generic / clinical assessment within a time limited period and reassessment of complex need and plan care to ensure individual patient care needs are met within a range of community and acute care settings
-Undertaking technical and fundamental nursing treatments for patients with complex healthcare needs.
-Administering where appropriate medication to patients with complex healthcare needs consistent with protocols, standards and legislation.
-Providing psychological and emotional support to patients and carer's
- Building and maintaining excellent working relationships with Yorkshire Ambulance Trust, A&E and Social Services to facilitate the most appropriate care pathways for patients

As a successful candidates you will be assisting in the development and provision of strong clinical leadership to develop a fast response home assessment and intervention service which will include both face-to-face and telephone consultations.

To be qualified for this role you will be RGN 1 with a degree appropriate to this field. Ideally you will be a nurse prescriber, with a teaching and assessing course, an masters degree is also desirable.

The ideal person for this role will have had extensive clinical experience in A&E and/ or Primary Care, Case management experience and knowledge of medico-legal issues affecting nurse practitioners.
