Hays social care have social work assistant jobs within the substance misuse team.
Overall Objective:

Support social workers in carrying out their duties within a Care Management Team

Job Function & Responsibilities:

*Advising clients about available resources;
*Liaising with the client to arrange appointments;
*Following up enquiries
*Carrying out reviews
*Visiting the client at home (community setting);
*Adhering to legislation and refreshing knowledge when required
*Report writing
*Maintaining and updating client records
*Working in partnership with other care and health professionals.
Relevant Training/ Qualifications: NVQ 2 Health and Social Care
